1. Entry Procedure: You complete & sign this form. Send us a cassette or CD with 2 songs you anticipate  

   to use in the competition if you’re selected to compete. Mail it to: Corey Susz, 603 SR 6W Apt.5, 

    Tunkhannock, Pa. 18657 by  August 01, 2008     

2. Registration Fee:  A one time non-refundable Fee of $5.00 per person is required. This fee is the only fee

    that will be collected .  This Fee dose not guarantee  a spot in the semi-finals or the final’s .

3. Disputes: The Kiwanis Wyoming County Fair hereinafter, “KWCF” is  authorized to resolve all disputes.

4. Performance:

      A. Each act entered in the contest must perform solo. 

      B. KWCF will provide the sound system but each  contestant must provide cassette or cd with 

           Pre-recorded music. NO LIVE Bands. I may play my own guitar. 

      C. Each act must perform LIVE, Vocally ,with pre-recorded music or with an instrument. NO special 

           lighting, props may be used

      D. Performance time will be  NO  more than 4 minutes.

      E. All performances must be done in good taste. Music or lyrics may not be offensive. 

           I understand  that I will be disqualified immediately if I become involved in any

           situation the would unfavorably reflect on KWCF or anyone  providing a prize.

      F.  Age restrictions will be 12 yrs. of age and older.

      G. Contestants that perform there own music and lyrics may receive extra points from judges. 

            Anyone that claims to be using original music and  in fact they are not  will be

           disqualified. KWCF requires proof of original music.         

      H. Judges will award a score of 0-10 in each of these categories:

           Vocal Ability, Stage Presence, Talent, Errors, Originality of Song. 

           The judges will award 0-2  points for original material.

           In the event of a tie, point given in  the talent category, will be the tie breaker.

           If still tied, the vocal ability  point will be the next tie breaker category. 

       I. The top three in point's will do a second song. The judges will then pick a winner, after

           the second round. 

5. Promotional Authorization:

            By submitting this entry form I here by grant  KWCF the right to utilize the act’s  name, likeness, 

            image, voice, appearance, and performance in perpetuity worldwide for advertising, trade or any

            other lawful promotional purposes whatsoever, in any and all media now known or thereafter     

            developed, without compensation. I grant  KWCF, officers and employees the right to broadcast,

            market, sell, and otherwise distribute my name, image, likeness, voice, appearance and performance,

            either in whole or in part for commercial, non-commercial television, close-circuit exhibition, home 

            video, distribution, internet, or any other purpose, KWCF, officers and employees in their sole 

            discretion may determine. I  hereby give all clearance, copyright or otherwise, for the use of my 

            name, likeness, image, voice, appearance embodied in my performance presentation.

6. Assumption of Risk:

             By submitting this entry form I assumes all risks of property damage, or bodily injury 

             (including death) that might occur during the contest. I agree not to bring KWCF, officers,   

             employees  and sponsors any claim for property damage or bodily injury (including death) that 

             might occur during the semi-finals or finals.
